You wake up and know something is missing. Not the hours. The depth. That quiet, restorative plunge that leaves your head clear and your body set for the day. Scientists now say losing that particular phase of sleep as you age could matter far more than we thought.
What the research looked at
Researchers followed hundreds of people from the Framingham Heart Study to probe whether declines in slow-wave sleep — the deep, restorative stage of the night — predict later-life dementia. Slow-wave sleep is the third stage of the roughly 90-minute sleep cycle. It typically lasts 20 to 40 minutes each cycle. Brain waves slow. Heart rate and blood pressure fall. The body and brain take on repair tasks, and memory-related systems consolidate new information.
The team analyzed polysomnography records collected from the same participants on two separate nights, once in the mid-1990s and again around the early 2000s, then tracked dementia diagnoses through 2018. That approach lets researchers look for change over time instead of relying on a single snapshot.
Key findings and numbers
Across roughly 17 years of follow-up, 52 participants developed dementia. The big pattern: slow-wave sleep declined steadily from age 60, with the steepest drop between about 75 and 80 years old. When the investigators compared the two sleep studies, they found that for every one percentage point per year loss in slow-wave sleep, dementia risk rose by about 27 percent. When the outcome was specifically Alzheimer disease, the risk climbed to roughly 32 percent.
These are relative risks based on observed associations. The study also linked low slow-wave sleep with other factors known to affect brain health, including cardiovascular disease, sleep-impacting medications, and carrying the APOE epsilon 4 gene variant associated with Alzheimer disease.

The study authors are careful to stress further research is needed to determine whether interventions that increase slow-wave sleep will reduce dementia incidence. Still, the notion that a changeable aspect of sleep could alter long-term brain outcomes is compelling.
