Imagine gaining more than a decade of clear thinking simply by changing three habits in middle age. That is the striking association emerging from a decades-long study that tracked more than 12,000 adults and measured how long they lived without dementia.
The research, published August 5, 2026 in Neurology Open Access and led by investigators including Josef Coresh, MD, PhD of NYU Grossman School of Medicine, compared dementia-free survival across groups defined by three vascular risk factors measured once in midlife: elevated blood pressure, diabetes, and a history of smoking.
- High blood pressure, defined as readings above 140/90 mmHg or the use of blood pressure medication
- Type 2 diabetes as recorded at baseline
- Current or former smoking history
Put simply, people who entered the study without any of these three risk factors remained free of dementia for far longer than those who had all three. The difference: roughly 30.1 years versus 17.5 years of dementia-free survival, a gap approaching 13 years.

Decades of data, a single measurement, and a strong signal
The study enrolled 12,409 participants who were dementia-free at baseline and had an average age of 56. Investigators followed them for about 26 years on average. During that time, 3,008 individuals developed dementia while 5,238 died without ever receiving a dementia diagnosis.
Why should this matter beyond the headline number? The study highlights how vascular health in midlife appears tightly linked to cognitive trajectories decades later. High blood pressure can damage small blood vessels in the brain. Diabetes alters metabolism and may promote inflammation. Smoking accelerates vascular injury and oxidative stress. Each pathway can chip away at brain resilience long before memory problems become obvious.
Still, the team cautions that the study shows association, not proof of causation. Only a single assessment of risk factors was available at the start, so researchers analyzed long-term outcomes based on that one snapshot rather than changes over time. Even so, the size of the cohort and the long follow-up strengthen the finding that midlife vascular risk is an important target for public health.
Patterns also differed by sex and race. Women tended to live longer without dementia than men across every risk profile. For participants with all three risk factors, women averaged 18.1 dementia-free years and men 16.6 years. Racial differences were present as well: White participants had more dementia-free years than Black participants at every level of vascular risk. For those with all three risks, White participants averaged 19.6 years vs 16.0 years for Black participants. These disparities point to complex interactions among biology, access to care, socioeconomic factors, and lifetime exposure to health risks.
Practical implications for prevention
The message is straightforward and urgent. Interventions that prevent or control high blood pressure, reduce diabetes risk, and eliminate smoking in middle age could meaningfully extend the years people live with intact cognition. That does not mean dementia would never occur, but delaying its onset by a decade or more would have enormous consequences for individuals, families, and health systems.
What might that look like in practice? Widespread blood pressure screening and treatment, programs that focus on healthy weight and blood sugar control, and robust smoking cessation services are all proven strategies. Complementary habits such as regular physical activity, healthy diets, and social engagement also support brain health and should be part of public messaging.
